Open issues report - Ability to view all open and in-progress issues together

Currently in the open issues report you can only view open OR in-progress issues. To do a full analysis we need to be able to see all open and in-progress issues together. Ideally the field would be a multi-selection on the report.

Add/Update/change COTI Link in Salesmen Left menu

Currently the link on the salesmens left menu "Customer Order to Invoice > Incomplete Deals" takes the salesman to his deals in COTI that Deal Details is not yet green. Salesman also need a link to any "Change Requests" assigned to them.

Add field on lost sale report to show Price premium both before and after the VAO adjustments

Reason Notes Column from "Requests" Export

It would be helpful to see the notes/reasons text from a users request as a separate column when viewing all the requests so we have the ability to export that list and view accordingly.
