Add field on lost sale report to show Price premium both before and after the VAO adjustments

Note Chain should show in descending order based on date/time

Ability to set up Extras that calculate their value as a percent of Net/List

We currently add an extra called a Service Pack which is intended to cover interest costs and other misc. costs. We would like to be able to add this extra and have it calculate as a percent of net or list.

Reason Notes Column from "Requests" Export

It would be helpful to see the notes/reasons text from a users request as a separate column when viewing all the requests so we have the ability to export that list and view accordingly.

Create ability to cc: others or select multiple individuals on Issues Created.

There are often times when we would want the Service Manager, as well as someone more specific to see when new Issues are generated, and today we can only send to one of them through the default assignment matrix.
