Support for DICO and other additional PSE fields

Additional data is now received and transmitted to Caterpillar in regards to PSE leads.

When SalesLink POSTs lead data, the following fields need to be additionally included
1. Sales rep number for dealer-originated opportunities (dealerSalesRepNumber in the POST call)
2. Opportunity closed date (opportunityClosedDate in the POST call)
  1. This is sent only when the opportunity is in a closed state.
When SalesLink GETs PSE data, we must parse these additional fields:
